Visitors to my Website
When someone visits http://www.faykalapoda.com/, a third party service, Squarespace, is used to collect standard internet log information and details of visitor behaviour patterns. I do this to find out things such as the number of visitors to the various parts of the site. This information is only processed in a way which does not identify anyone on a personal level. I do not make, and do not allow Squarespace to make, any attempt to find out the identities of those visiting my website. If I do want to collect personally identifiable information through the website, I will be up front about this. I will make it clear when I collect personal information and will explain what I intend to do with it.

Maintaining your personal information
I collect or use the personal information to provide my services such as:
● Personal Identification Information: Name, date of birth, gender, home address, email address, phone number, and other contact details (GP contact details, next of kin details including any support networks and emergency contact details.
● Health Information: Medical history, medical conditions, medical test results, psychological assessments, disabilities, treatment records, medications, notes from our sessions together and any other information relevant to your mental health.
● Payment Information: Debit/Credit card details and billing information if applicable.
● Communication Data: Records of your correspondence with me, including emails and phone calls.
Your personal information held will be used solely by Eftychia (Fay) Kalapoda and will not be shared with any third parties, except where I request your consent to do so, or under special circumstances where I am legally required to do so.
The reason I keep your personal information
Our professional registration requires me to keep information about my clients and the work that we do. I cannot offer you services unless you allow me to keep data about you and our work together, and I have a legitimate interest for keeping your data. I am bound by the ethical and practical rules set by the professional regulatory bodies.
I use the data I collect for three reasons:
(1) to provide you with services,
(2) for billing and processing payments,
(3) to help prevent serious harm.
Some of my clients may feel vulnerable at times in their lives, and be at risk of harming themselves or be at risk of harm to / from others. In these circumstances, I need to be able to communicate effectively with other services such as GP surgeries or emergency care services, to keep these clients safe. This involves sharing of personal information on a need to know basis.
Sharing your personal information
I hold information about each of my clients and the therapy they receive in confidence. This means that I will not normally share your personal information with anyone else. However, there are exceptions to this when there may be need for liaison with other parties:
● If you are referred by your health insurance provider, or otherwise claiming through a health insurance policy to fund therapy, then I will share appointment schedules with that organisation for the purposes of billing. We may also share information with that organisation to provide treatment updates.
In exceptional circumstances, I might need to share personal information with relevant authorities:
● When there is need-to-know information for another health provider, such as your GP.
● When there is a legal requirement (including court orders) to collect, share or use the data;
● When the information concerns risk of harm to the client, or risk of harm to another adult or a child. I will discuss such a proposed disclosure with you unless we believe that to do so could increase the level of risk to you or to someone else (the public interest to collect, share and use the data overrides the public interest served by protecting the duty of confidentiality).
Please note that I do not share your data with third parties for marketing purposes.
How long your personal information is kept for
Your data is kept throughout your therapy and, in line with professional guidance, for 7 years after the work has ended.
Security of your personal information
I will put in place appropriate safeguards (both in terms of the procedures and the technology I use) to keep your personal information as secure as possible.
Your rights to your personal information
You have the following rights regarding your personal information:
● Your right to access your personal data. You have the right to ask me for copies of your personal information.
● Your right to rectify any inaccurate or incomplete data.
● Your right to erase your personal data (the “right to be forgotten”).
● Your right to restrict processing of your personal data.
● Your right to data portability.
● Your right to object to processing. Any objections you make to the processing of your data will be stored against your record on my system so that I can comply with your requests.
● Your right to withdraw consent – When we use consent as our lawful basis you have the right to withdraw your consent at any time.
